The Medi-Bug Beetles are a Potara-style fusion from the Demon Realm that anyone can afford, with no dragon needed. Two players who eat a matching pair of beetles fuse, whatever their races.

Getting the beetles

Buy a Pair of Medi-Bugs (Beetle) for 16 emeralds from the bug traders among the Demon Realm Villagers. Their trades restock once per in-game day.
Or get one from Bulma's repeatable Medi-Bug Market sidequest, unlocked during the Daima Saga: bring her emeralds and gold ingots and she orders a batch of Medi-Bugs, a Beetle pair included, once an hour.
Right-click the pair to split it into two identical Medi-Bug (Beetle) items sharing the same pair ID. Give one to your partner.

Fusing

Each player eats their beetle. You must both eat beetles with the same pair ID within 30 seconds of each other and be within 20 blocks. The first player sees "Waiting for your partner to eat the other beetle…".
The two players are pulled together into the Potara pose and fuse. The player who ate first is the leader and controls the fused body.
If your partner doesn't eat theirs in time, your beetle wears off.

Result

Bonus stats scale from ×1.25 to ×2.0 (STR, SKP and PWR), like the Fusion Dance.
Like the Potara Earrings, any races can fuse, there is no stat-gap limit, and upgraded Androids can fuse too.
The fusion lasts the full server duration. When its timer runs out, the leader goes on Fusion cooldown, which only blocks the Fusion Dance.

Other Medi-Bugs

The same traders sell other Medi-Bugs that don't fuse: the Ant (restores 85% of your health, ki and stamina), the Rhino (30% less damage for 45 seconds) and the Worm (50% more form mastery for 5 minutes). See Items.
