Demon Realm Villager
Demon Realm Villagers are the residents of the demon villages of the Demon Realm, dressed in Demon Realm outfits. About 40% of them are bug traders who sell Medi-Bugs.
Where to find them
Demon villages generate on the Third Demon World: a plaza surrounded by 3–5 adobe houses, 1–2 farms, adobe-brick paths and lantern posts. Each house has 1–2 villagers. Every village also has a Time Patroller on its plaza.
Medi-Bug trades
A bug trader sells 2–3 of these offers for emeralds. The other villagers don't trade.

Trades restock once per in-game day when you trade with the villager. The beetle fusion is explained on Medi-Bug Beetles. Besides the bug traders, Bulma's repeatable Medi-Bug Market sidequest sells a batch of Medi-Bugs (a Beetle pair included) for emeralds and gold ingots, once an hour.
