Infobox for form pages, built on Module:Infobox (DRUID). Every parameter is optional; empty rows are hidden. Labels are translated automatically from Module:Labels/data. Without image it shows File:<page name>.png when that file exists, or the placeholder otherwise, so uploading File:<page name>.png is enough to replace it.
